Parents of Athletes:
Please read the following important information regarding sports physicals (Pre-Participation Medical Exam/History).
Pre-participation physical exam date should be performed on or after March 10th to be valid for the following school YEAR through May 31, of that year. (Example: March 10, 2023 good thru May 31, 2024).
Items Needed For All Athletic Tryouts
Completed Register My Athlete registration
Completed Participation consent and Liability Disclosure Document (done within Register My Athlete).
UHSAA Pre Participation Examination/ Medical History and Consent Forms
For Freshman: 9th grade Participation form
No more than 1 "F" or "I" in the previous terms grades
2.0 or greater GPA for most recent completed term
Less than 5 detention hours prior to first team competition
School Transfer approval if student attended a different school the previous year
These documents may be downloaded with the links above or picked up in the Athletics or Main Office at Springville High School.
It is also important to realize that if your student is selected for a team, they must also meet the grade requirements set by the team and have all fees paid prior to the first competition.
How to Pay Activity Fee
Fee’s need to be paid before the first game/activity/meet, in order to participate.
There are 2 ways to pay your fees:
In person at the school Finance Office
Online at MyScoolFees.com
Connect to www.MySchoolFees.com
Login using your E-mail or Create an Account
Select your student using Student ID
Select the sport and pay the amount
The Funds will be deposited at the school and you will receive an E-Mail receipt
If your student is a 9th grader, you will need to come to the school to pay for them. There will not be a way for them to pay online.
Please do not call and try to pay the fee over the phone! The district does not allow this due to fraud and higher fees charged to the school.
There is a $2.00 processing fee associated with paying with a credit card whether you pay online or at the school. Paying with cash/check has no fee.
If you cannot pay the fee due to financial hardships, you can make an appointment with someone in administration to see if you qualify for a fee waiver. The coach cannot waive your fees, only an administrator can do that.
